Who We Are

Care that starts with a simple idea

Homestead Health started with a simple idea: older adults should not have to leave home to get good medical care. For too many families, a single health scare means an ER visit, a hospital stay, and a long road back. We exist to change that.

We are a 501(c)(3) nonprofit organization. That means every service we provide is driven by mission, not margin. Our team of physicians, nurse practitioners, and care coordinators come to your home or meet with you virtually so that care fits into your life, not the other way around.

We serve older adults across the Phoenix metro area. Whether you are managing a chronic condition, recovering after a hospital stay, or simply want a care team that knows your name, Homestead Health is built for you.

Aging at home means staying in your own space, surrounded by what you know, close to the people you love. It means keeping your routines, your independence, and your dignity.

But aging at home safely takes more than good intentions. It takes a care team that shows up, listens, and stays ahead of problems before they become crises. That is what Homestead Health does.

We do not measure success by how many appointments we fill. We measure it by how many ER visits we help our patients avoid. By how many families sleep better knowing someone is watching over their parent. By how many older adults tell us they feel like themselves again.

Chris has spent the past two decades working with older adults and navigating the gaps that often appear in our healthcare system. After years of developing hospice and home-based care programs, he founded Homestead Health with a straightforward purpose: to help seniors remain in the place they know best, HOME, and ensure they receive the support they often struggle to find elsewhere. Chris takes time to listen, provides honest and practical guidance, and helps people understand their health without shying away from meaningful conversations. His focus is to help patients age in place with clarity, choice, and dignity.

Elyssa supports patients and families through counseling, care navigation, coordination, and emotional guidance. With more than a decade of experience caring for individuals with chronic and complex conditions in both hospital and home-based settings, she understands how overwhelming navigating healthcare, caregiving responsibilities, and evolving medical needs can be for patients and their loved ones. Trained at the VA, Elyssa is skilled in coordinating care across fragmented healthcare systems, connecting families with resources, and advocating for patients when it matters most. Her compassionate and practical approach focuses on reducing stress, improving quality of life, and helping patients remain safe, supported, and independent at home whenever possible.

Family Nurse Practitioner

Cherise is a board-certified Family Nurse Practitioner with over 20 years of nursing experience across hospice, palliative care, and primary care. She is experienced in managing both acute and chronic conditions with a patient-centered approach that emphasizes prevention, healthy lifestyle, and care built around each individual. She is known for building strong, trusting relationships with her patients while delivering efficient, high-quality care in both in-person and telehealth settings.

Jorge has a strong background in operations, provider relations, and community outreach, and specializes in building strategic partnerships with assisted living communities, home health agencies, hospice organizations, and senior care professionals. Prior to transitioning into mobile healthcare services, Jorge gained valuable experience in the pharmacy field for over 15 years, helping shape his patient-centered approach and understanding of healthcare coordination. He now plays a key role in expanding access to primary care, palliative care, transitional care, and telehealth services across the valley. Known for his relationship-driven leadership style, Jorge is passionate about connecting healthcare providers and senior living communities with resources that improve patient outcomes and quality of care.
