Our Mission

Home is where healing begins

"We measure success in ER visits avoided and families who can finally exhale."

Homestead Health exists to help people live comfortably and safely at home. Aging in place, surrounded by family and the things you love, consistently produces better outcomes than any clinical setting can offer.

Our physicians, nurses, and care managers bring the full scope of medical care to your doorstep. Preventive wellness, complex chronic disease management, care coordination: we handle all of it so you never have to navigate the system alone.

As a 501(c)(3) nonprofit, mission drives every decision we make. When an older adult tells us "I got to stay home," that is the outcome we are working toward.

About us
Getting Started

How it works

Three steps. No referral, no prior authorization, no runaround.

1

Call or Write to Us

Reach out by phone or our contact form. A real care team member will pick up, listen to your situation, and help you figure out if Homestead Health is the right fit. No scripts, no pressure.

2

We Come to You

A qualified provider visits your home. We review your medical history, walk your living environment, and build a full picture of what support will actually help. The whole visit happens on your turf.

3

A Plan That Stays With You

Together, we build a care plan that fits your life. We stay involved and adjust as your needs change, so you and your family always know what comes next and who to call.

Who We Serve

Two groups who need us most

Whether you're the one receiving care or the one making sure a parent gets it, Homestead Health is here.

Seniors aging at home

You value your independence. You want to stay in your home, close to the memories and the people you love. Our care team brings primary care, chronic disease management, and wellness visits to your door. You stay safe, well, and in charge.

Adult children caring for a parent

You're balancing work, your own family, and real worry about a parent who may live miles away. We become the medical partner you can trust to keep watch, coordinate care, and call you when something changes. You stay informed without carrying it all alone.
